The
following guidelines have been developed to help us serve you better. Please
take a moment to review them before signing your lease agreement.
1)
Equipment
is carefully packed and insured against damage in shipping. Any damage to
shipped items must be reported immediately so that we may file a claim with the
shipper. Failure to notify us of damage may leave you
responsible for repair charges.
2)
Your
equipment should be received in excellent condition. If equipment is not
operable due to damage or malfunction, we must be advised immediately so we may
provide a replacement. We cannot be responsible for lost time unless we are
promptly notified.
3)
Please pack
equipment carefully for return shipment. Please do
not ship items without a box! If
possible, save the original shipping cartons and use them for return shipment. Return
shipments must be insured to
avoid shipper liability in case of damage in shipment.
4)
We cannot
be responsible for lost time due to programming errors and/or lack of expertise
on the part of the user. Please review your instruction manual thoroughly before
use of equipment.
5)
Your lease
term begins on the day equipment is received and ends the day it is shipped back
to Environmental Leasing. If lease extension is required, please contact us.
6)
As noted in
Article 2 of our Lease Agreement, extensions to this lease will be invoiced from
the ending date of the original lease and at the original rate unless negotiated
otherwise.
7)
Please
ensure that all items listed on the packing slip are received. All items must be
returned, including accessories. (Excluding tubing & bottles, which may be
discarded).
8)
Please clean all equipment
thoroughly before return shipment. A cleaning fee of $50.00 will be assessed if
items are not returned clean.
